MINIMUM DRIVER QUALIFICATION GUIDELINES

1.) Must meet all Federal Motor Carrier Guidelines.

2.) Must have a valid Commercial Driver License with proper endorsements in the state of residence.

3.) No current license suspensions or revocations.

4.) Minimum age 23.

5.) Two years of verifiable tractor-trailer experience.

6.) No more than 3 moving violations in the past 3 years.

7.) No more than 2 moving violations in the past 12 months.

8.) No serious or disqualifying traffic violations in the past 5 years as follows:

a. Excessive speeding, involving any single offense for any speed of
15 m.p.h. or more above the posted speed limit.

b. Reckless driving, as defined by state or local law or regulation,
including but not limited to the offense of driving a motor vehicle in
willful or wanton disregard for the safety of persons or property.

c. Improper or erratic lane changes.

d. Following the vehicle ahead too closely.

e. Driving while intoxicated or under the influence of drugs.

f. Hit and run, leaving the scene of an accident, or failure to report
an accident.

g. No more than 3 preventable accidents in the past 3 years.

h. No preventable accidents involving a fatality, or bodily injuries
treated away from the scene, or disabling damage to a motor vehicle within three years.

9.) No more than 2 jobs in the past 12 months.

I further agree, in the event that I am offered employment by the company, as a condition to that employment, all disputes that cannot be resolved by informal internal resolution which might arise out of my employment with the company, whether during or after that employment, will be submitted to binding arbitration in lieu of any Federal or State investigative, administrative or legal proceeding. I agree that such arbitration shall be conducted under the rules of the American Arbitration Association. This application contains the entire agreement between the parties with regard to dispute resolution, and there are no other agreements as to dispute resolution, either oral or written.
